TCMP is actively bidding on a contract where Clinician/Therapist services are needed to support the Department on Disability Services (DDS), Developmental Disabilities Administration (DDA) in Washington, DC for the next 5 years. DDS, DDA coordinates home and community services for more than 2,000 individuals. Clinicians/Therapists shall aid in supporting the Agency’s commitment to providing improved health care services and quality of life for individuals with intellectual and developmental disabilities in the District of Columbia (District).

Required Qualifications:
  • Shall have two (2) years of experience working with individuals who have intellectual and developmental disabilities or other specialized graduate training and experience relevant to the unique needs of people with developmental disabilities and
  • Possess current clinical knowledge serving underserved populations who are culturally, linguistically and racially diverse   

Responsibilities include, but are not limited to the following:
Providers shall work directly with the DDS/DDA residential and day services provider to complete the following, as requested:
  • Evaluate the Health Care Management Plan and/or Health Risk Management Plan and other health information in the record for individuals supported by DDS/DDA within the first 60 days;
  • Visit the individual in all support settings to evaluate, recommend or provide additional services required by the individual’s Health Risk Management Plan (in the clinician’s area of specialty) within the first 60 days;
  • Assist DDS interdisciplinary team which includes the Service Coordinator to address any direct support and/or clinical support needs for the individual(s) to facilitate the interventions outlined in the Health Care Management Plan;
  • Provide on-going clinical assessment, follow-up and intervention in collaboration with DDS staff (as needed) for individuals identified with high medical or behavioral health needs and/or risks each month, and of all individuals supported /assigned by the agency every three (3) months;
  • Immediately report to DDS/DDA any clinically significant concerns identified in the course of the on-going oversight process;
  • Meet with DDS/DDA staff, individual family members, guardians, attorneys, advocates or other natural support persons as needed to facilitate effective health care outcomes and/resolve outstanding issues; and Participate in individual risk assessments, utilizing tools identified or approved by DDA annually for assigned individuals.
